Whether mental health services are covered will vary depending on your particular plan. Prior to your initial session, please reach out to your insurance provider in order to determine the costs associated with mental health counseling, including your deductible, and any copay you might owe. Use the customer service number located on the back of your insurance card to do this. Having clear expectations about what your counseling services will cost you can help us maintain a healthy working relationship.

1. Are outpatient mental health services covered under my plan?

2. Is Indira Kalmbach and/or Kalamazoo Counseling Center, PLC in-network under my plan?

3. Do I have a deductible? If so, how much is it? Have I met my deductible yet?

4. What is my patient copay per visit under this plan?
